Case Management Program

Project FIND is proud to partner with Encore Community Services to deliver essential case management services for homebound older adults in Manhattan. Funded by the NYC Department for the Aging, this program will assist up to 325 older adults across Community Boards 4 and 7 — including Chelsea, Clinton/Hell’s Kitchen, Manhattan Valley, the Upper West Side, and Lincoln Square.

Why This Matters

Now more than ever, homebound older adults need help accessing vital services. Since the pandemic, the risks of frailty, isolation, and cognitive decline have grown. Our dedicated case managers connect clients to resources that help them age safely at home and stay engaged with their communities.
