HEALTH CARE PROXY CLINIC

As part of the Annual Health and Wellness Fair, we will conduct a Health Care Proxy Clinic from 9:30am to 11pm.

 All Senior Citizens should have a Health Care Proxy. By having a Health Care Proxy, you can designate someone to make health care decisions for you if you can no longer make the decisions for yourself.

Anyone in need of a Health Care Proxy can meet with a volunteer attorney and sign a health care proxy. There is no charge. If you want to sign a health care proxy, you must come to the meeting with the name, address and phone number of the person you are naming as your health care agent. If you want to name an alternate agent, you should also bring to the meeting the name, address and phone number of that person as well.
